ATB Problem to Add Command Options

Well,here's the detail..i think no one understand bout my meaning..Command in the menu is growing, but does not alter the function, for example, I added the command "Drain" in one character, but what happened is not a new command but the command "Skill", which changed its name but the same function, whereas the command "skill" to "guard" , And "Guard" to "item", while "Item" can not choose ... Can you please help me?
